If you are interested in joining the Oregon Traumatic Brain Injury Consulting Team, please download and complete the TBI Team application materials.

Materials for the applicant include:

  • Applicant Cover Letter
  • Application
  • TBI Team brochure

Materials for the applicant’s supervisor include:

  • Cover letter describing the TBI Team to your supervisor
  • Statement of Support for Staff Participation, for your supervisor to complete and return to you

TBI Team membership will be considered once we receive your completed application, and signed form of supervisor support for your participation.

Athletic Concussion Training FREE Online

The ACTive program is available FREE online. It only takes 20 minutes to get the tools and information you need to help keep young players safe!

Coaches play a critical role in reducing the risks associated with concussion in young athletes. The athletic concussion training program (ACTive) is based on the the recommendations of leading sports concussion experts. A clinical trial funded by NIH showed that coaches who view the ACTive program have significantly greater knowledge of concussion symptoms and have increased confidence in making decisions about concussion management.
